The Office of the Kendall County Coroner encourages hospice and palliative care agencies to pre-register all anticipated home deaths to ensure a smooth reporting process upon the patient's passing.  By completing the pre-registration, you assist our staff in verifying the information that will be collected at the time of death (such as demographic information) as well as greatly minimizing the chances of delays or follow up calls from an investigator when the death occurs. To make this process easier for your staff, we have created a fillable PDF form that can be submitted electronically (by email) or faxed 24 hours-a-day.

NOTE: Pre-registration of a patient does not relieve the medical professional of their responsibility to notify the coroner upon determination of death. The Coroner requires ALL deaths occurring within Kendall County to be reported immediately by calling 630-553-4200 (24/7/365).
